Our story

We started Resilient because the participants we cared most about were the ones being turned away.

Resilient Life Care is a family-run NDIS support provider. Louisa runs the Lumi Support side · coordination, plan management, the navigation work. Michael runs Resilient · direct support workers, complex care, hospital transitions, the hands-on care that happens in someone's home every day.

When we started, we said yes to everyone. Two hours a week. Twenty hours a week. Twenty-four hour care. That meant a lot of admin and a lot of context-switching, but it taught us what NDIS participants and their families actually go through.

Now we focus on the participants and families that need us most. Hospital transitions. Complex care that other providers say no to. SIL placements that need a clinically literate team that won't leave halfway through.

Hospital Transitions

From hospital bed to home or to supported accommodation. Discharge planning, in-home setup, complex-care handovers from hospital teams. We work with discharge coordinators and treating teams directly. Two of our largest current clients arrived this way.

Supported Independent Living

SIL placements with a team that stays. Standard and high-needs. Single 1:1 and shared 1:2 / 1:3 ratios. Active overnight or sleepover. Melbourne, Brisbane (including Caboolture), Sunshine Coast, Gold Coast.

Motorcycle accident, multi-trauma. Discharged from a Melbourne metro hospital. We started planning before he left the ward and had a 24-hour roster ready on day one. Three years on, same lead carer, same family.

Tier 1 · Complex behaviour

"They actually understood Level 3."

Adult with autism Level 3, intensive PBS plan. The previous provider escalated everything to behavioural incidents. We trained the team properly, built a routine that holds, and have not had a hospital admission since.

Tier 1 · Stroke recovery

Brisbane, Caboolture catchment. Stroke recovery, discharged with significant support needs. We handled the transition from hospital to private accommodation, including the housing logistics his family couldn't carry.

Common questions

Are you NDIS-registered?

Yes. ABN 75 672 596 217. Registration current to 26 May 2028. We are listed on the NDIS Commission provider register.

What complex care do you support?

Tracheostomy, ventilation, severe dysphagia, PEG feeding, complex bowel care, seizure protocols, intensive positive behaviour support. We hold the clinical and behavioural training in-house and bring in registered nurses where the plan funds them.

Do you work with hospital discharge teams?

Yes. We come in before the participant leaves the ward. We have ongoing relationships with discharge teams across Melbourne metro hospitals and the north Brisbane area.

How do you handle 24/7 rosters?

Lead carer plus backup carer per participant. Active overnight or sleepover depending on the plan. Same team week to week. We do not sub-contract or use agency casuals on Tier 1 plans.

Where do you operate?

All of Melbourne. Brisbane (including Caboolture and the north Brisbane area). Sunshine Coast. Gold Coast. We will travel further for the right plan.

Can I switch providers to you?

Yes. NDIS plans give participants the right to choose providers. We handle the handover paperwork and coordinate with your existing coordinator if you have one.
